At the time of the 1992 challenge, Carroll County Public Schools had a formal process for dealing with book complaints, but it was not originally followed.1The original parent complaint about The Floatplane Notebooks was verbal – via telephone call – and the principal of the school took action without receiving a written complaint.2

Although the Carroll County School Board no longer has the complaint form that was filed during the time of the case, the form below is an blank complaint form3 that was used by Carroll County Public Schools at the time of the challenge in 1992.

This form asks a range of questions that documents the issue, explores the complainant’s knowledge of the work in question, and asks about why he/she objects to its use.
